If I had any questions regarding my mother's care, the nurses always took the time to answer any questions. Immaculate Mary Center for Rehabilitation & Healthcare is a very-large nursing home located in Philadelphia, Pennsylvania. We gave this nursing home a C overall, which is not a bad score. This is comparable with the city grade in Philadelphia, which is a B-. Based on our ratings, there are certainly far worse nursing homes out there. We were also pleased to find that this place received consistently decent grades in each of our categories.

A resident was found by a licensed practical nurse lying on the bathroom floor after an unwitnessed fall. The LPN noted that the resident seemed slow to respond and generally unwell. After notifying the physician of the residents condition and vital signs, the nursing staff received orders to order a STAT EKG and to notify the physician of any changes to the residents condition. The nursing staff did not complete the EKG until 14 hours after they had received orders from the physician. They also did not notify the physician of changes in the residents vital signs.

The nurses were nice, some worked there for a long time, which to me is a sign of a good place to work. Because it is located in the city, there isn't a large outdoor nature area, which to me is a minus.Overall, I liked the facility, I heard good things about it from the resident I know that lives there. I also know from that resident that sometimes they encounter short staffing and "it's not handled well right away", so that is a minus for my review for this facility. My father was placed here on the 3rd floor with Alzheimer's disease. I was told by the director that they do not offer "one-on-one care". They transferred my father to the hospital twice in only 3 1/2 weeks & on the 2nd time refused to take him back.
